Our services include pre-employment physicals, DOT physicals, school entrance exams, travel health consultations, TB testing, and health clearance documentation.
Bring the required form and deadline to the visit. Completion depends on the examination, available records, testing requirements, and whether further evaluation is needed.

Consultation fees, deposits & cancellation terms
Peptide education consultations are free, with no deposit. Hormone and weight-loss consultations are $200, credited toward treatment if you proceed. Labs are charged separately.
For hormone and weight-loss consultations, our front desk collects a $50 booking deposit by phone. It is part of the $200 consultation fee, not an additional charge. For other new-patient appointments and procedures, the front desk confirms the applicable $50 deposit and applies it toward the visit.
Please give at least 24 hours’ notice for cancellations or changes. Applicable booking deposits are non-refundable and are forfeited for late cancellation or non-attendance. A $75 fee applies to physical exam appointments canceled with less than 24 hours’ notice.
