Do not enter symptoms, diagnoses, medications, treatment history, or other sensitive medical details in a callback request. The form is not monitored for emergencies. Call 911 for an emergency.

Information Collected Automatically

If you choose optional analytics, Umami may process limited technical and usage information such as:

• Browser, device, language, and screen information
• Public page paths, service or topic identifiers, and broad website interactions
• Referring domain and campaign parameters
• Approximate location derived from network information

Optional analytics can run across public website pages. Sensitive care paths are replaced with a generic path before analytics delivery. We exclude owner-only and staff-workspace routes, form-field contents, locally saved service selections, and user-provided identifiers from analytics.

Cookies and Local Storage

We use the following types of data storage:

• Essential browser storage: Keeps your theme preference, saved service selections, consent choice, and a random first-party visitor and campaign record on your device. The campaign record preserves the first paid source for up to 90 days and may accompany a callback request. Sensitive care destinations are stored only as "sensitive," not as an exact path.
• Optional analytics: If you select "Accept All," Umami may receive privacy-filtered page and interaction measurements together with campaign fields. Declining analytics does not remove the first-party campaign record used to connect a callback request with the source that brought it to LifeSpann.

Selecting "Essential Only" prevents Umami from loading. Clearing browser storage removes locally saved choices and the first-party campaign record.
