Cancellation Policy
A $50 non-refundable deposit is required to confirm your appointment for all new patient consultations and procedures — our front desk will reach out to collect it after you book. The deposit is applied toward your visit cost. We require at least 24 hours' notice for cancellations or changes. No-shows or late cancellations will forfeit the deposit. A $75 fee applies for physical exam appointments not canceled with 24 hours' notice. By booking an appointment, you agree to these terms.

Psychiatric care, available in the same Gilbert clinic
Horizon Peak Health is a separate psychiatric practice located inside LifeSpann. Canybec Sulayman, PMHNP-BC, AGPCNP, is a psychiatric and adult-gerontology nurse practitioner who provides evaluation, medication management, and interventional psychiatry through Horizon Peak Health.
The shared location gives patients a clearer route between physical and mental health care. When it is clinically appropriate and you authorize it, the two practices can coordinate around the same care plan.
